Employee Health Screenings

Employee health screenings promote overall health and wellness for employees to decrease chronic disease, improve employee health and productivity, and reduce long-term healthcare costs.

Personal Health Profile

The Personal Health Profile (PHP) is a comprehensive Health Risk Assessment (HRA) designed to assess the current health status and potential future risks of an individual. The PHP includes a detailed questionnaire including health history and risk profile. The questionnaire can be completed online or hard copy in English or Spanish.

Venipuncture screening

The blood draw includes a lipid panel total cholesterol, HDL, LDL and triglycerides (Heart Disease screening), Hemoglobin A1c (Diabetes Screening). If an employee has a result in the critical range our provider will contact them within two business days to discuss. Prisma Health offers additional selected lab testing based on employer needs.

Biometric screening

Biometric screening includes blood pressure and body fat percentage evaluation by measuring height and waste circumference to calculate Relative Fat Mass Index (RFM).

Screening results education

Laboratory test results education is provided to employees by our health educators.

Company aggregate report

The Group Health Profile summarizes the percentage of employees, by gender, race and age, and outlines key health risks for specific diseases. This health snapshot will compare the health of your workforce to others in the market and identify specific health risks present in your workforce. Our business health team will assist in developing a plan to address the high-risk employee groups and will recommend education sessions and disease management programs. The Group Health Profile will also allow you to trend, over time, the improvements in the health of your workforce. The aggregate report is generated and presented on an annual basis but is available by request prior to the end of the year.

HR Insight

Once the aggregate report is presented, your company representative has access to HR Insight, an internet-based financial modeling tool, to calculate return on investment (ROI)/savings based on lifestyle, behavior and health status change.


Your Prisma Health wellness specialist will assist in marketing the screenings and education sessions.


Additional Health Screening Options

Personal Health Report

Each employee who participates in a health screening receives a customized Personal Health Report. This reports the results of the screenings, comparative statistics and information as well as providing tips to improve health in a particular area. The report is comprehensive yet easy to navigate and ideal to take with them when employees see their providers.

Individual Wellness Coaching

Wellness coaching is designed to improve the health and well-being of a business population by providing individualized education and support. This program is based on a team approach to care including nurse practitioners, nurses, registered dietitians, and wellness coaches. The team works collaboratively to provide care that meets an individual where they are; giving participants not only the knowledge they need, but also the tools and support to leverage knowledge into action. We identify and coach employees based on their health conditions or behaviors as identified by the health assessment and screenings (listed in order of risk): 

  • Prediabetes/Diabetes
  • Waist circumference
  • Hypertension
  • Depression
  • Tobacco use
  • Primary care provider status

The number of wellness visits recommended are dependent on the risk factors identified. Individuals with higher risk values, typically with the need for clinical intervention, will meet with a nurse practitioner, nurse or registered dietitian. Individuals with lower risk values who will benefit from lifestyle change or resource navigation, will meet with a wellness coach. Individuals may meet with more than one wellness professional depending on their care plan and needs.
