Often, people who are chronically ill need more than medical help when they go home from the hospital. Community Care aims to assist patients who lack the ability to complete basic nonmedical tasks by pairing them with a group of volunteers to provide assistance for their identified needs.
Our volunteer teams can help with a wide variety of activities, including:

- Social outings
- Phone check-ins

Our Community Care teams are made up of volunteers from the local community, who want to help make a difference in the lives of their neighbors.
Volunteering during COVID-19
As we find ourselves navigating a new reality due to COVID-19, we are still committed to making a positive impact in our community. Our team is committed to not only caring for our program participants but also maintaining the safety of our volunteers to the best of our ability. We have adopted the following measures to aid in that effort.
- Volunteers will be provided with face masks and hand sanitizer.
- Volunteers do not have to enter the program participant’s home.
- Any time a volunteer feels unsafe they should leave immediately and report any concerns to program staff.
- The Community Care team will meet via phone or video conferencing if they are not comfortable meeting in person due to social distancing concerns.
