Spinal Cord Injury Program

Our Spinal Cord Injury Program focuses on providing education and facilitating patients' independence in the home and community.

Entry into the outpatient program begins with a physician referral. When a patient arrives for an assessment, our team will begin the evaluation process. The services the patient receives will depend on the referrals made to the therapy department based on the patient’s needs.

Our team will meet monthly to collaborate and coordinate the goals and assess concerns or obstacles to the plan. Input from the patient and family will be provided through the case manager, family conferences (as needed), as well as any member of the team.

Referrals to appropriate support agencies will be initiated and medical equipment will be secured. Post-discharge regimes will be established through a collaborative relationship with the patient/family, community services, the family physician, and the follow-up physician visits.
